Registered Nurse- Days Flex days and evenings Northern NJ MMC OMC CMC

• Conduct comprehensive clinical assessments to determine hospice eligibility, prognosis, disease progression, functional decline, symptom burden, and patient/family goals of care.
• Obtain and review clinical documentation and collaborate with the Hospice Medical Director to determine hospice eligibility and the appropriate level of care, including General Inpatient (GIP) eligibility.
• Assess patient learning needs and evaluate progress toward goals.
• Meet with patients and families to explain hospice services, levels of care, and the admission process.
• Facilitate hospice consents, admissions, certifications, and initial care planning.
• Complete GIP admissions, including assessments, documentation review, physician collaboration, admission paperwork, and care plan development.
• Complete discharge assessments as indicated

Required:
1. Graduate of an accredited Nursing Program.
2. Current state licensure as a Registered Nurse.
3. BLS certification.

Preferred:
1. Bachelor’s degree in nursing.
2. All ASN must complete their BSN studies within five years of their start date.
3. 1+ year of experience in Nursing or hospice.
