Job Summary: The Intensive Case Manager provides intensive and comprehensive case management services in Spanish to formerly homeless residents housed in the Broadway Sansome apartments, (families and single adults) and the newly opened senior building, Philip C Chin Commons; both buildings provide supportive housing to these residents with various complex medical, mental health and substance use histories. This position requires a motivated and resourceful person, who shows sensitivity to helping address the needs of a diverse, ethnic, and formerly homeless population.
